Distributor Having Yearly Turnover of Up to Rs 12,00,000
Supplier Having Yearly Turnover of Up to Rs 12,00,000Turnover of Up to 12 lacs/annum

Documents Required to Obtaining FSSAI registration in Kerala

The list below reflects the type of documents required to be attested with the application for new, conversion, renewal and modification of food license.

  • Form B duly filed and approved by the application via signature.
  • The processing facility layout manifesting the dimension & operation-wise area allocation. (Legal compulsion for manufacturing facilities)
  • List of serving partners/Directors/ executive members of society/ trust with full address & contact particulars (compulsory for companies).
  • Name and detail of machines along with their power rating, installed capacity
  • Identity as well as residential proof of proprietor/Partners/directors/authorised signatory.
  • List of food items proposed to be manufactured and processed at manufacturing facilities.
  • Authority letter reflecting the name & address of accountable individual nominated by the manufacturer along with alternative accountable person manifesting the rights vested with them by assisting the officers in verification, a gathering of sample, packing and dispatch.
  • Analysis report of potable water proposed to be used in food items from the certified lab.
  • Proof of premises’ possession.
  • Partnership deed
  • No-objection certificate and manufacturer’s license.
  • FSMS plan or certificate.

Documents required for Renewal of License

  • Form B duly filed and approved by the authorised signatory
  • Declaration Form – For Delhi or Himachal Pradesh.
  • Annual audit report/FSMS plan
  • Form IX: Nomination of Individual by an entity along with the Board Resolution

Documents required for Modification of food License

  • Form B duly filed and approved by the authorised signatory
  • Any modification in documents furnished during the grant of the preceding license
  • Certificate or FSMS plan
  • Details pertaining to the technical person accountable for handling the operation
  • Form IX: Nomination of Individual by an entity along with the Board Resolution
  • The schematic of the processing facility manifesting the dimensions in metres & operation-based area allocation
  • List of Food Category
  • NOC as well as the proof of the license from the manufacturer
  • Declaration Form – For Delhi or Himachal Pradesh

FSSAI’s Fee Structure for the Grant of Food license

The charged for the license renewal is based on the number of years selected.

For Duplicate of License

The charge for obtaining the duplicate license has been capped at 10% of the application license fee.

The charge for the license modification is fees for one year per license modification.

How to Apply for FSSAI registration in Kerala?

An applicant seeking FSSAI registration in Kerala[1] needs to file an e-form in the online portal called FOSCOS. Once you log onto this portal, open the “new application” option enlisted on the right section of the home page. Selecting this option will unlock the descriptive e-form on your screen where you need to facilitate the following details:

  • State where the business is situated
  • Annual turnover
  • Nature of business; i.e. retailer, manufacturer, marketers, etc

In the next window, you will be prompted to upload the essential documents regarding business registration, PAN, partner’s details, premises details so on and so forth.
