Documents Required to Obtaining FSSAI registration in Kerala

  • Form B duly filed and approved by the application via signature.
  • The processing facility layout manifesting the dimension & operation-wise area allocation. (Legal compulsion for manufacturing facilities)

  • Name and detail of machines along with their power rating, installed capacity

  • Form B duly filed and approved by the authorised signatory
  • Declaration Form – For Delhi or Himachal Pradesh.

  • Form IX: Nomination of Individual by an entity along with the Board Resolution

Documents required for Modification of food License

  • Form B duly filed and approved by the authorised signatory
  • Any modification in documents furnished during the grant of the preceding license
  • Certificate or FSMS plan
  • Details pertaining to the technical person accountable for handling the operation
  • Form IX: Nomination of Individual by an entity along with the Board Resolution
  • The schematic of the processing facility manifesting the dimensions in metres & operation-based area allocation
