Please note that the notary\u2019s approval of this affidavit is important to make it legally enforceable. <\/li><li>Information to be appended in the Eating House License Affidavit<\/li><li>Name and address of the applicant <\/li><li>Business place name and address<\/li><li>Validity of trade license <\/li><li>Designation of the applicant in the company <\/li><li>Contact details of the applicant <\/li><li>Duration for which business place is running<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Confirmations_made_by_the_Applicant_in_an_Eating_House_License_Affidavit\"><\/span>Confirmations made by the Applicant in an Eating House License Affidavit<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Through the Eating House License\nAffidavit,\nthe applicant confirms that &#8211;<\/p>\n\n\n\n<ul><li>The business place has been registered under the respective <strong>Municipal Corporation Act<\/strong><sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Municipal_Corporations_Act_(India)#:~:text=Municipal%20Corporations%20Act%20helps%20in,resolution%20of%20local%20self%2Dgovernment.\"><strong>[1]<\/strong><\/a><\/sup>, or the renewal application for the same is in process. <\/li><li>The business place has been or has not been charged for the offence(s) committed in the past.\u00a0 <\/li><li>The business place has a required approval in place for meeting the pollution norms.<\/li><li>The KYC of all serving personnel has been done as per prescribed guidelines. <\/li><li>The business place has been registered under COPTA guidelines and has a dedicated smoking zone, if any.<\/li><li>The fire NOC from the State pollution board has been secured.<\/li><li>The proper fire fighting equipment has been installed within the business premises.<\/li><li>The layout plan of the business place has been approved by the competent authority.<\/li><li>There is a fire evacuation plan in place to deal with emergency situations.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Other_Documentations_for_an_Eating_House_License_Application\"><\/span>Other Documentations for an Eating House License Application <span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Apart\nfrom the said affidavit, the following documents also act as a key enclosure to\nthe Eating house license application &#8211;<\/p>\n\n\n\n<ul><li>Identification evidence such as a\nPAN card, etc.<\/li><li>Residence proof such as Passport,\nVoter ID, Aadhaar card, etc.<\/li><li>Tenancy evidence such as Sale Deed,\nLease Deed, or Rent Agreement<\/li><li>Self-attested NOC of the business\nplace owner <\/li><li>Copy of Trade License <\/li><li>Certificate of Incorporation (COI),\nin case the business place is registered under the Companies Act, 2013<\/li><li>Name of the serving Directors\n&amp; current form 32<\/li><li>The nomination of an authorised\nsignatory on the company\u2019s letterhead <\/li><li>Board resolution reflecting\nmembers\u2019 consent for obtaining eating house license <\/li><li>Food license granted by the FSSAI<\/li><li>GST certificate <\/li><li>Name of the serving Directors and\nform 32 <\/li><li>Approval from the respective\nlabour department <\/li><li>Fire safety certificate granted by\nthe respective fire department (mandatory in case of where a seating capacity\nis equal to or greater than 50 <\/li><li>Workforce details such as name, contact\ndetails, and phone number<\/li><li>The Provisional License granted by\nthe pollution control board<\/li><li>Approved site or building plan\nattested with three copies plus fire safety norms cited on map (applies to\neating house whose seating capacity is less than 50<\/li><li>Police clearance approval <\/li><li>Request for providing amusement\nservices like Live Band Discotheque, Live to customers <\/li><li>Invoice of CCTV system involved\nwithin the premises <\/li><li>Photos of the dining zone within\nthe proposed eating house&nbsp; <\/li><li>Any other documents, if required.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Process_of_securing_an_Eating_House_License\"><\/span>Process of securing an Eating House License <span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>The\nprocess of obtaining an Eating house license in a given state is as follows &#8211;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Step 1: <strong>Application submission<\/strong><\/h3>\n\n\n\n<p>The applicant is required to fill the prescribed\napplication as advised and submit it under the applicable authority.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Step 2: Submit the documents in a standard format <\/strong><strong><\/strong><\/h3>\n\n\n\n<p>Next, submit the mandatory documentation in the\nstandard format along with the affidavit as mentioned above. <\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Step 3: Approach <\/strong>Additional Commissioner of\nPolice Licensing<strong> <\/strong><\/h3>\n\n\n\n<p>After online application filing, visit the Additional\nCommissioner of Police Licensing of the respective state for an application and\nreports of the vetting. <\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Step 4: Regular follow-up<\/strong><\/h3>\n\n\n\n<p>Your application, along with documents, remains\nunder scrutiny for a while. In the meantime, you are required to keep the tap\non the progress of the same through a portal and respond to authority\u2019s queries\naccordingly, if any.
